Submission of “Residence Status・Contact Information Confirmation Sheet”

We would like to inform you that we have made changes to the submission method of “Residence Status・Contact Information Confirmation Sheet” due to the impact of coronavirus disease (COVID-19).

【How to submit】

  • Please submit it to Office of International Students via e-mail as an attachment.
  • The title of your e-mail should be “【Confirmation Sheet】”.
  • You must include the information below in your e-mail.
  1. The name of your Faculty / graduate school
  2. Your full name
  3. Student ID

【Required】

Please obtain the form from the "Related Links" below.After entering required fields and attaching the images of both sides of your residence card to the specified area of the form, please send it to OIS by e-mail.

For those who do not obtain a residence card

  • If you are expected to obtain a residence card by the deadline, please submit this after obtaing the card and by the deadline.
  • If you are expected "NOT" to be able to obtain a residence card by the deadline, please write "Not obtain yet" in your e-mail and submit a confirmation sheet by e-mail. Also, after you have obtained the residence card, please resend the confirmation sheet with the attached image data of your card by e-mail.
